applications submitted through this online questionnaire will be
considered for evaluation.
All questions must be answered. All information will
be treated confidentially. We remind you that you will have four
(4) hours to complete this questionnaire. Therefore, before
submitting online, you may print a blank version of this
questionnaire by clicking on Printable Form at top right.

Please note, SIGA does not support the following
ventures:
- Political organizations or events.

Religious organizations (except for those with
non-faith based purposes)
- Travel for individuals or groups

Capital campaigns - we do not support capital
campaigns, but can assist with fundraising initiatives i.e.
silent auctions
- Individuals

Promotional marketing or advertising items; including
naming rights on buildings.
If you are a successful applicant, within three months after
your event, you may be required to fill out a Post Event
Questionnaire. Within this time an email will be sent to
you with a hyperlink to the questionnaire. These Post Event
Questionnaire are mandatory in order to be considered for future
sponsorship. Please complete the survey as completely as possible.
Attach any promotional materials for your event as a
"sponsorship".
